Bring your documents to your visit or send them to our clinic.

• Unexpired Photo ID
• Birth certificate (minors)
• Vaccination records (if available)

You will have a short visit where you will complete any required blood work for the exam.

Within one week of your blookwork you will. be scheduled to meet with a provider. The provider will review your lab results and perform the physical.
